We require an institutional evaluation form and three (3) additional evaluation forms. We require 1 letter from a pediatric dental faculty member. Especially as many schools report P/F instead of grades, we rely on our pediatric dental colleagues, as they are most familiar with what a residency entails, etc. For those who have graduated and are currently in practice, two (2) letters from educators are required (with at least 1 pediatric), and they do not have to be from your own dental school. We do not recommend that you submit letters from colleagues, employers, or non-dentists.

        ******

You will recieve a confirmation email from our Program Coordinator as receipt of your completed PASS application -- a completed application includes all of your evaluation letters. Our first rounds of confirmations will be sent July 15. For submissions after July 15, please allow 5 business days to confirm. If you do not receive this receipt after 5 days, you can contact Pedsdentresidency@nationwidechildrens.org or 614-722-6024.
